This enrollment application is the first step in the IIA registration process into the Certificate Program. It is designed to provide your instructor with important background information that will be kept confidential, and kept as part of your student file.

Upon approval and acceptance into the program, your IIA Certificate Program materials kit will be shipped to you via insured USPS Priority Mail. A follow-up phone orientation meeting will be planned with you after receipt of course materials. Laraine will review with you the layout of the course, including the course forms and answer any questions you may have.
IIA Refund Policy: If you are not satisfied with the IIA Certificate Program for any reason, you may return it within 7 days, less 10% processing fee. Return the kit to your instructor via UPS or USPS insured shipment, with all the educational materials in the original box and all blending supplies unopened and in excellent condition.
